Assists patients and/or patients’ families in understanding and accessing community resources to assist with barriers in areas, including, but not limited to, medical, behavioral health, education, and employment.Works within the requirements of department programs, as well as applicable IHS Grants for Native Americans, and fulfills community health objectives as dictated by these programs and/or grants. Partners with psychiatric and allied teams, as well as Clinic health professionals, in the diagnosis, creation, and maintenance of the patient care plan. Patients may be self-referrals, referred from medical centers, or referred internally from the Clinic health professionals. Pay Range: $70,920 - 88,649, DOE + BenefitsĪt the direction of the Behavioral Health Director, and in his/her absence, the Licensed Clinical Social Worker performs clinical assistance to behavioral health patients and their families.
